Completing the Customer Service Survey

The Customer Service Survey seeks to identify the major barriers people with diverse disabilities experience when attempting to access everyday goods and services. The Survey also seeks to capture your accessibility stories, as well as your ideas and suggestions on what can be done to make your customer service experiences easier.

The BARRIERS section contains 75 barriers that people with disabilities may experience. After each statement you are asked to identify the degree to which the barrier affects you, as well as how frequently you experience this barrier. Check the boxes that best reflect your experiences. At the end of the formal questions, you can add other barriers or issues that you feel are important.

The SUGGESTIONS section provides you with the opportunity to give us your ideas on how access to quality customer service can be improved.

Please also complete the demographics section. Understanding a bit about you will help us to communicate the importance of the different issues to businesses and organizations.Lastly, we would love to hear your accessibility stories. You can attend a focus group, send us your story as part of the questionnaire, or both.

Please note that all responses to the survey are confidential. Your responses will be combined with others and no information that relates to any identifiable person will be released.

The survey takes approximately 20 to 25 minutes to complete and is available in 3 ways: it can be completed online, in accessible Word format or as a fill-able PDF. The Word and PDF formats can be emailed back to us OR mailed to the address provided at the bottom of this page.
